Incident: An occurrence, condition, or situation that arises in the course of work that results or could result in injury, illness, damage to health, or fatality.
- UNMC employees, students, and volunteers are required to complete an Incident Report for all accidents/incidents involving injury.
- Injuries/ accidents/ near-miss incidents that involve patients and/ or occur in Nebraska Medicine clinical areas: Report using the Nebraska Medicine Shout Out for Safety, RL Reporting System.

- Unsafe conditions or "Near-Miss" incidents that did not result in injury, did not involve patients, or occur in clinical areas may be reported to UNMC EHS using the Near Miss/Potential Hazard Form.
- Individuals may also report Compliance Issues or concerns by calling the UNMC Compliance Hotline at 1-844-348-9584.
*Hotline callers may remain anonymous if desired.
Care and Additional Reporting
- Emergency: Serious injury that requires immediate and/ or extensive medical attention, immediate threat to life, health, and/ or campus security.
- Call Public Safety Dispatch at 402-559-5555 and/ or go to the Emergency Department for care.
- Severe Injury and Fatality Reporting: Managers and supervisors must immediately contact Public Safety Dispatch whenever there has been a work-related fatality or a severe work-related injury of an employee.
- Employers must report work-related fatalities within 8 hours after learning of the incident.
- For any in-patient hospitalization, amputation, or eye loss; employers must report the incident within 24 hours after learning of the incident.
- Call Public Safety Dispatch at 402-559-5555 and/ or go to the Emergency Department for care.
- Known or potential exposure to biologics (blood/ tissue/ biohazardous agents/ contaminated equipment):
- Perform immediate first aid as appropriate (wash, flush, wrap, etc.)
- Contact the OUCH Pager Immediately: 402-888-OUCH (6824)
- Enter a call- back number after the tone. An on-call nurse will call back for information and will provide further instruction.
- Incidents involving biological agents in research must be reported within 24 hours to the IBC using the Adverse Events Form.
- Non- life threatening, minor injury:
- Perform first aid as appropriate (wash, flush, wrap, etc.)
- If you need additional care or are not sure, contact UNMC/ NM Employee Health.
- Please call ahead to the EH clinic for minor injury care and services. A provider may not be immediately available, and care may need to be deferred to alternate providers such as Comp. Choice or the Emergency Department. If appropriate, an appointment will be scheduled at the clinic. Calling ahead will save employees time and help the clinic team triage care needs.
