Whether using the Clinical Research Center or the Clinical Trials Management System (CTMS) there is a centralized intake process to aid you in building your studies. Please complete the Study Intake Form.
For study start-up to begin the following documents must be attached to your intake form:
- Protocol
- Consent Template
- Budget Template
- Editable Contract
- Contract Questionnaire
- Investigators Brochure/Instructions for Use for Devices
- Patient Facing Documents
If any of these documents are not applicable, an explanation must be provided.
Studies will not be placed in a queue for teams to begin start-up activities until all required documents are received.
