Fees for Budgeting

As of July 1, if your budget includes advertising, you may be charged a fee for service. An invoice can be provided to assist with billing back to the sponsor. The fees for each service are here.

Interest Form

Creating an interest form helps streamline the process of an interested patient signing up for the study. You can link your form to a flyer QR Code, a Facebook Ad or as a short link in an opt-in message. Microsoft forms and Redcap are approved to use for interest forms. Here is a link to the template.
