Required Immunizations

All incoming, on-campus students must document receipt of immunizations prior to registration. Online students (except students whose training includes clinical settings and patient contact) are exempt.

Additional immunizations may be required before working in clinical or community partnerships. Please review immunization guidelines set by UNMC Student Health for additional immunization requirements and due dates.

Access submission instructions.
Submit records through MyRecords, using your UNMC NetID and log in information.
Submit all immunization records simultaneously. Incomplete forms will not be accepted.

Required Immunizations and Documentation

Immunization	Documentation
Varicella (Chicken Pox)	Provide record of Titer showing immunity, OR Document two varicella vaccinations, OR Obtain vaccinations, administered at least four weeks apart
Hepatitis B	Provide record of three-dose series and a positive Hepatitis B Surface Antibody Titer OR Provide a Hepatitis B Surface Antibody Titer showing immunization AND Provide record of first HepB dose prior to registration
MMR (Measles/Mumps/Rubella)	Provide record of two MMR vaccinations, OR Provide record of Titer showing immunity for all three parts of MMR OR Obtain vaccinations, administered at least four weeks apart
Polio	Provide record of most recent polio vaccine, OR Provide record of Titer showing immunity, OR Consent of no travel to Africa or Asia within the past five years
TB Skin Test	Provide record of current TB test within one calendar year of start date OR Provide IGRA within current year If you have ever tested positive in the past please review the full policy.
Tdap Tetanus/Diphtheria/Pertussis	Provide record of Tdap within last ten years, OR Obtain vaccination prior to registering
Flu	Not an admission requirement Annual flu vaccination encouraged for anyone encountering patients; free clinics offered to students on campus during academic year Required for all College of Nursing students May be required for other programs and clinical sites
COVID-19 Vaccine	Document COVID-19 vaccinations OR Decline vaccine
