Patient Eligibility
Guidelines
Those interested in being seen at the SHARING, GOODLIFE, and VISION Clinics must meet the following criteria:
- Age 18 or older
- No health insurance
- Annual household income below 200% of the Federal Poverty Level based on 2024 guidelines.
- Proof of income may be requested that includes at least one of the following:
- Current pay stubs, last federal income tax return, documentation of other income sources
- NOTE: Proof of income is encouraged but not required to be seen - this can be discussed on a case-by-case basis
Those interested in being seen at the RESPECT Clinic must meet the following criteria:
- All are welcome to be seen at no cost regardless of income or insurance status
