Study Intake

First Steps

Whether using the Clinical Research Center or the Clinical Trials Management System, there is a centralized intake process to help build your studies. Please complete the Study Intake Form.

For study start-up to begin, the following documents must be attached to your intake form:

Protocol
Consent Template
Budget Template
Editable Contract
Contract Questionnaire
Investigators Brochure/Instructions for Use for Devices
Patient-facing Documents

If any of these documents are not applicable, you'll need to provide an explanation.

Studies will not be placed in a queue for teams to begin start-up activities until all required documents are received.
