What?

Use data to help your team understand how well your fall risk reduction program is working to reduce fall risk in your hospital. Data can be generated from sources such as gap assessments, fall events, post-fall huddles, auditing of fall risk reduction interventions, and team meetings.

Why?


Knowledge and understanding of how well your fall risk reduction program is working creates buy-in and allows the opportunity for your fall risk reduction team to create and adjust action plans to improve desired outcomes.

How?

Your fall risk reduction team should regularly review results of your hospital’s gap analysis, results of process audits, discuss knowledge generated through post-fall huddles, look for trends in fall event reports, and monitor total, injurious, and unassisted fall rates over time.
