Providers: Refer patients to Engage Wellness exercise program

EngAge Wellness in the Home Instead Center for Successful Aging encourages health care providers to refer patients to EngAge’s “Exercise is Medicine” program.

EngAge Wellness employs a whole-person wellness approach to optimal aging for individuals ages 50 and older or Medicare-eligible. The program focuses on the six dimensions of wellness:

  • Social;
  • Physical;
  • Intellectual;
  • Spiritual;
  • Emotional; and
  • vocational.

Patient perks

Exercise is Medicine is a national initiative by the American College of Sports Medicine that encourages health care providers to assess patient’s exercise habits as a vital sign at each patient visit and recommend each patient to be physically active.

Providers are encouraged to refer their patients 50 years of age or older to UNMC’s EngAge Wellness “Exercise is Medicine” provider referral program to receive:

  • Free two week participation in the EngAge Wellness program;
  • A functional fitness assessment;
  • An individualized fitness program designed by a degreed exercise science professional;
  • One-on-one orientation to their fitness program;
  • An option to participate in group movement classes;
  • A referral to other appropriate services to reach patient wellness goals; and
  • Communication back to health care provider regarding patient’s participation and progress.

Services for providers

Providers would receive communication regarding each patient’s participation and progress through the free trial period, and beyond, should the patient decide to join the Exercise is Medicine program.
