A note for those with university health insurance

If you are enrolled in the UNMC health insurance as of Jan. 1, you will need two different cards.

The card from Blue Cross Blue Shield should arrive at your home shortly — if you have not already received it — and will replace any card you had from 2010 or earlier.

It will be pre-printed with your name and ID number. The names of your covered dependents will not be printed on the cards.

You also will need a separate card for CVS Caremark for your prescription drugs.

Please go to the University of Nebraska Benefits Website at www.nebraska.edu/benefits to download and print your paper CVS Caremark prescription drug card.

To find the prescription card:

  • Click on Benefits Forms (listed under Other Benefits Information);
  • Then look under Prescription Drug Program and click on Caremark Identification Card.

Your CVS Caremark ID number is a 9 digit number and consists of your personnel ID number preceded by zeroes:

  • If your personal ID number is 5 digits – it will be preceded by four zeroes (i.e. 000012345).
  • If your personal ID number is 6 digits – it will be preceded by three zeroes (i.e. 000123456).

Your personnel number can be found on your paystubs in Firefly as well as the confirmation statement you received for NUFlex 2011.
