Wellness Wednesday – Information for new health insurance enrollees

Are you newly enrolled in the UNMC health insurance program as of Jan. 1?

If so, you will need two cards — one from Blue Cross Blue Shield for your health care appointments, and another from CVS Caremark for prescription drugs.

The card from Blue Cross Blue Shield should be arriving at your home shortly, if you have not already received it.

It will be pre-printed with your name as well as that of every covered member. If you have a dependent who is not listed on the card, please contact the benefits office.

Please go to the University of Nebraska Benefits Web site to download and print your paper CVS Caremark prescription drug card.

You will find the prescription card when you click on “Benefits Forms” (listed under other benefits information) and then look under “Prescription Drug Program,” and click on “Caremark Identification Card.”

Your CVS Caremark ID number is 9 digits, and consists of your personnel ID number preceded by four zeroes (if your personnel number is five digits) or your personnel ID number preceded by three zeroes (if your personnel number is six digits), i.e. 000012345.

Your personnel number can be found on your paystubs as well as the confirmation statement you received for NUFlex 2009.
