Sign up is now open in Apollo for February fit testing. The next appointments will be held May 2020. Check with your manager if you are not sure if you are required to be fit tested.
The fit testing will occur during these dates at the following locations:
- Feb. 17-21: Nebraska Medical Center, Learning Resource Center 1, basement level of Storz Pavilion
- Feb. 24-25: Nebraska Medical Center, Learning Resource Center 1, basement level of Storz Pavilion
- Feb. 26: Bellevue Medical Center, Conference Room D
- Feb. 27: Bellevue Medical Center, Conference Room C
- Feb. 28: Internal Medical Associates in Grand Island
Sign up in Apollo for your appointment by logging in, selecting “Search for Trainings” on the left-hand side, then typing “fit testing.” Plan to arrive 10 minutes before your appointment. If you are late, you may be asked to reschedule.
Fill out and sign the applicable FROST form and bring it to your appointment
- If you’ve previously been fit tested by FROST: Medical History Review
- If you’ve not previously been fit tested by FROST: Respiratory Protection Medical Form
Fill out and sign the Consent for Treatment Form and bring it to your appointment
For UNMC employees only, in addition to Consent to Treatment, fill and sign the following forms, and bring it to your appointment
Other notes:
- Registration will be closed 24 hours prior to each session, so be sure your appointment is in Apollo. Walk-ins will be accepted only if there are any openings at that time.

- If you are unable to keep your appointment, please cancel it. This ensures the next person on the waitlist can be informed of the opening
- Males must be clean shaven in the area where the mask will make contact with their skin
- If pregnant, fit testing cannot be completed at this time.
