More than one hundred community members, patients and Nebraska Medicine colleagues recently commemorated Brentwood Village's new clinic location at 8604 Giles Road. On July 12, a ribbon-cutting ceremony was held, along with an open house from 5:30 to 7 p.m.
By moving to the new facility, the practice space has expanded to include 15 exam rooms, five consult rooms and a procedure room. One of the consult rooms is dedicated to telehealth, allowing for the community clinic to connect patients with a multi-discipline of services if immediately necessary during a regular office visit.
What's Happening in Primary Care
Another new clinic offering primary care services will open on Sept. 18 at 8343 S. 168th Ave. Nebraska Medicine – Chalco will have a similar layout as Brentwood Village clinic. Look for more information about an open house event in upcoming editions of NOW.
"The added conveniences for patients in the new clinic are really remarkable," says Connie Monjaraz, clinic manager. "The old clinic at 84th and Giles Road had been used 21 years prior to this move and with the addition of services and providers we had outgrown the space."
With a strong focus on Patient Centered Medical Home (PCMH) model of care, the team of providers partner with patients and family members to provide quality medical care for acute illnesses, chronic disease management and preventive health care. The PCMH model puts the patient at the center of everything the clinic does, providing multiple services all in one location. Multi-disciplines at Brentwood Village include Pharmacy, Behavioral Health, Diet/Nutrition and Social Work.
"The providers and staff at Brentwood clinic feel fortunate to have a new and larger facility to work in," said Susan Schwerdtfeger, MD, medical director, Brentwood Clinic.
