Employees must complete an online consent form before they attend free flu vaccination clinics.

Also, employees will need to bring their employee IDs to the clinics.
The clinic schedule for employee flu shots is as follows:
- Monday, Oct. 31 — 7 to 9 a.m. and 11 a.m. to 1 p.m., Storz 1-4;
- Monday, Nov. 7 — 1 to 3 p.m., Private Dining Rooms A, B and C;
- Tuesday, Nov. 8 — 7 to 9 a.m., Storz 6-8;
- Wednesday, Nov. 9 — 1:30 to 3:30 p.m., Storz 1-4;
- Friday, Nov. 11 — 7 to 9 a.m and 1 to 4 p.m., Storz 1-4;
- Monday, Nov.14 — 2:30 to 4 p.m., Private Dining Rooms A, B and C;
- Tuesday, Nov. 15 — 7 to 9 a.m., Storz 6-8;
- Wednesday, Nov. 16 — 7 to 9 a.m., Private Dining Rooms B and C; and
- Friday, Nov. 18 — 7 to 9 a.m., Storz 1-8.
