Leadership & Assistance Opportunities

MMI's individual and family resource coordination staff provide support and referral services.
Care Coordinators at the Munroe-Meyer Institute are trained professionals in human services disciplines. They are knowledgeable about state and national service systems for individuals with disabilities and their families.As an individual with a disability or a family member working with our team, you can expect to receive information based on your specific health care needs. Our goal is to ensure that you have access to the opportunities and resources to help you reach your full potential, regardless of the nature of your disability.
Support
Our coordinators have extensive training and experience in the area of individual and family rights regarding disabilities and special health care needs across the lifespan. On occasion, coordinators are contacted when an individual’s rights are in question.
In these situations we:
- Provide information and resources on the rights of individuals with disabilities and their families.
- Refer individuals or families to agencies whose work specifically involves protecting the rights of persons with disabilities.
- Provide community training on topics related to disability rights, services and supports.
Referral
Referrals to our individual and family resource coordinators are made by MMI staff, other community professionals and the individuals and families themselves.
A referral includes:
- Assessment of the current status of the individual or family concern.
- Referral information on diagnostic and therapy services provided at MMI.
- Referral information on community and national service providers as well as disability specific resources.
- Development plans that enable the individual and family to obtain needed community services and supports.
- Assistance navigating service and support options for individuals or family members.

Care Coordination Program
