Care Coordination at Munroe-Meyer Institute
Our Care Coordination program consists of an interdisciplinary team of professionals, social workers, service coordinators, parent resource coordinators and interpreters.
A life changing diagnosis or event may leave you feeling out of control, alone and fearful. If you are seeking answers, a clinical diagnosis, services, resources, or you are looking to determine a plan of care – care coordination is available to assist you navigate the health care system.
Our program will collaborate and work with a multidisciplinary team to ensure continuity of care.
If you, your child, or family member is receiving services at Munroe Meyer Institute, ask one of your providers to refer you to the Care Coordination program.
Care coordination offers these services and assistance:
- Clinical consult and guidance
- Referrals for services and programs
- Needs assessment
- Peer-to-peer support
- Advocacy
- Community engagement
- Collaboration with local schools (K-12)
- Information about Medicaid, Medicare, private insurance and social security income
- Brief crisis counseling
- Community support groups
- Referrals for specialty or specific health and mental health care
- Health care navigation
Our Care Coordination program will utilize a strengths-based approach customized to your individual and family’s unique needs.
