Nebraska Geriatric Workforce Enhancement Project

Enhancing Healthcare Outcomes for Older Adults by implementing Age-Friendly Care using the 4Ms framework

What Matters Focuses on what Matters most, what brings the individual joy and what they cannot live without; coordinating advance care planning and ensuring the individual’s priorities and preferences are part of their treatment plans.

Medications Focusing on Medications means raising awareness about potential side effects while also working to reduce the number of medications taken whenever possible. Medications should not interfere with what Matters most, Mobility or Mentation.

Mobility means being able to move freely on one’s own or with help. Mobility is linked to staying physically fit and able to live on one’s own for as long as possible while “being safe, and independent. It means maintaining the ability to walk, stay balanced and avoid falls and injury.

Mentation or Mind. Staying mentally sharp and managing mental health help us live longer and fuller lives. That is why expert attention to the Mind—is an important part of well-being.  Focusing on Mind means supporting brain health and managing conditions like dementia, delirium, and depression, which affects one’s ability to think clearly and make decisions.

  • The Nebraska Geriatrics Workforce Enhancement Program (NGWEP – or Nebraska GWEP) aimed to improve the healthcare and healthcare outcomes of Nebraska’s older population by educating teams of primary care provider teams, patients, families, caregivers and health professions’ trainees.
  • NGWEP was a five-year partnership between UNMC (University of Nebraska Medical Center), Nebraska Medicine and primary care practices in Omaha and 13 rural counties in Nebraska, the partners listed above, and the University of Nebraska at Omaha School of Social Work
