Federal Verification Instructions for the Parent
Parents, follow the steps below to complete the verification process.
Complete the Parent Verification Worksheet for the appropriate academic year. You will be required to install the latest version of Adobe Reader on your computer to use this option. You will need to complete, print, sign, and submit the form using the instructions on the form.

Request an IRS Tax Transcript
Parents must request and submit an IRS Tax Transcript OR parents can return to the student's FAFSA and utilize the IRS Data Retrieval Tool in lieu of requesting an IRS Transcript. Not all parents may be eligible to use this tool. If you try to use the Data Retrieval Tool and the system says you are not eligible, you will need to request an IRS Tax Transcript (see below). THE IRS ACCOUNT TRANSCRIPT IS NOT THE SAME AS AN IRS TAX TRANSCRIPT, NOR CAN IT BE ACCEPTED BY THE OFFICE OF FINANCIAL AID. PLEASE REQUEST THE IRS TAX TRANSCIPT ONLY!
IRS Transcripts can be requested THREE (3) ways:
1. On-line request at IRS.GOV.
2. Complete and MAIL Form 4506-T - Request for Transcript of Tax Return to the following address: OR

3. Complete and FAX From 4506-T - Request for Transcript of Tax Return to 801-620-6922 (see page two (2) of instructions)
Mail downloads and supporting documents (Parent Verification Worksheet, IRS Tax Transcript, and all W2s to:

Parents need to put the student's name and UNMC ID or SSN on each document sent.
Students have 60 days to complete the verification process. If you experience delays in obtaining IRS transcripts, please notify our office by email. We will make a note in your file. As a courtesy, we will continue to send you a reminder letter every 30 days until your file is complete. File completion delays as a result of IRS processing will not result in reduced financial aid packages for those students who completed their FAFSA before the deadline.
UNMC reserves the right to collect additional information from you and/or your parents should the verification process result in questions regarding your financial aid application.
