Fall semester enrollment (May - December)
Application opens December 1st and closes March 1st
Spring semester enrollment (January - May)
Application opens June 1st and closes September 1st
HOW TO APPLY
Please follow the instructions below to apply for the next enrollment session.

In some cases applicants my use a separate preceptor and proctor. If necessary, please have your proctor complete this form.
The BSCLS DAO Admissions Committee shall consider each applicant who has submitted the Online Application and the Application Check List along with all required materials by the published deadlines above. The steps to completing the application process are detailed in steps 1 and 2 above.
All accepted students are required to complete a Background Check prior to attending classes. The School of Allied Health Professions reserves the right to revoke acceptance based on any discrepancies found on a background check.

Enrollment and Registration
Applicants will be notified of acceptance upon receipt and review of completed application forms, all required documentation, and official transcripts. This process takes approximately two to four weeks to complete after the application deadline. Accepted applicants will be approved for enrollment and provided with instructions and procedures for online program and course registration.
